Attention Colt Community: This is a 14-day reminder for school entry Immunization and Health Assessment Requirements for NC schools for the 2022-23 school year.

Kindergarten students and students enrolled in a NC public school for the first time are required to provide a certified copy of completed immunizations and an up to date Health Assessment. Below is a link to the health assesment form.
 
The assessment must occur within 12 months prior to the first day of school. The date of assessment must be on or after August 29, 2021.
 
7th grade students are required to have a Tdap and the Meningococcal vaccine.
 
12th grade students are required to have a second dose of the Meningococcal vaccine.
 
If you have uploaded documentation with online enrollment or turned in documentation in the office, please check to make sure it meets NC requirements. We must have this documentation on file by September 27th in order for your child to remain in school. If not received, your student will not be allowed to return on September 28th and will be excluded until it is provided.
